- Policy number or booking reference
- Name and date of birth of the claimants
- The contact address for mailing
- A day time telephone number
- Details of where you brought the insurance
- The date you brought your insurance
- The start and end date of your insurance policy
- The start and end date of your trip
- The date you booked your trip
- The destination of your trip
- The date of the incident giving rise to the claim
- The section under which you want to claim.
